Passport Applications

Please bring your complete passport application UNSIGNED and a copy of all required documentation https://travel.state.gov.  The passport office is unable to take passport photos or make copies of required documents at our facility.

  • Some states now issue digital ID documents (also known as mobile driver’s licenses or mobile ID’s).  We cannot accept these digital IDs when you apply for your U.S. passport.  You must continue to submit your physical, photo ID and photocopy of the ID.
  • If you present an out-of-state ID, you must present an additional ID.
  • If you are changing the gender marker in you U.S. passport, please refer to the https://travel.state.gov website and Selecting your Gender page.

FIRST TIME APPLICANTS:

Apply in person using the DS-11 form and provide the following documentation:

    • Original or certified copy of birth certificate (or original certificate of naturalization)
    • Photo ID
    • Passport photo (we are currently unable to offer photos on site)
    • Payment in the form of check or money order for passport fee (to be mailed in with application). Separate processing fee can be paid with a cashier’s or certified check, or card.

For Minors: If applying for a minor, both parents must be present with child and present their ID. If one parent cannot be present, Form DS-3053 “Statement of Consent”  must be completed, signed & notarized along with a FRONT & BACK copy of ID. If a parent or guardian has a court order declaring custody, an ORIGINAL court order must be submitted with the application.

Passport Fees

All passport fees are set by the U.S. Department of State. All payments to the U.S. Department of State must be in the form of personal check, cashier’s check or money order.  Each application submitted must have its own personal check, cashier’s check or money order. Temporary or counter checks are not acceptable forms of payment to the Department of State.

 Fees payable to the Cobb County Superior Court Clerk may be cashier’s check, money orders, or credit cards.  Payments made by credit card incur a 3.5% convenience fee from the service provider.

  1. ) APPLICATION FEE: A CHECK OR MONEY ORDER PAYABLE TO U.S. DEPARTMENT OF STATE
    • Write the name and date of birth of the applicant on the front of the check or money order

ROUTINE SERVICE:

  1. ) PROCESSING FEE: CASHIER’S CHECK OR CERTIFIED CHECK, DEBIT/CREDIT, OR MONEY ORDER – PAYABLE TO COBB SUPERIOR COURT CLERK
    • ROUTINE SERVICE: $35 charge PER application     
    • EXPEDITED SERVICE: $59.70 PER application (with overnight shipping — $35.00 application fee; $24.70 overnight shipping fee), $35 (without overnight shipping)

RENEWALS FOR ADULTS (PASSPORT WAS ISSUED AT AGE 16 OR OLDER):

You must renew by mail using Form DS-82 if your most recent passport meets all five of these requirements:

  1. Was issued within the last 15 years
  2. Was issued when you were age 16 or older
  3. Passport can be submitted with the renewal application
  4. Is undamaged (other than normal “wear and tear”)
  5. Was issued in your current name (or you can document your name change with an original or certified copy of your marriage certificate, divorce decree, or court order)

If any of the above statements do not apply to you, you must Apply in Person using form DS-11.

RENEWALS FOR CHILDREN AGE 15 OR YOUNGER

If renewing a passport for children age 15 or younger, you must re-apply in person using Form DS-11.

  1. Both parents AND child must be present when applying
  2. If one parent cannot be present, Form DS-3053 “Statement of Consent” must be completed, signed & notarized along with a FRONT & BACK copy of ID attached
  3. Original or certified copy of birth certificate is required (even with the previous passport)
  4. Parents photo identification
